Your group quote is dependent on the ages of all of the employees in your group to be insured. Click the button above to download the census form in Excel format. Once downloaded, complete the form with all of your employee information. Once completed, use the link below to upload your completed census form. Once we receive your completed census we can contact you with your quotes within 2-4 business days.
